1: They know where to look. That's why they don't. (Plausible deniability)
2: Those that have looked are not allowed to speak to those that would need to know (stay in your Silo)
3: And if #2 happens, plausible deniability is removed from #1. So there is a small reorg to regain the plausible deniability so everything can go back to BAU.

@OP Common sense says leadership is no good. Mainly because there are no leaders. Maybe explain what an internal audit is. Did they assess themselves and realize their own incompetence? Did they give themselves an action plan to have it corrected? Inquiring minds want to know.
