It looks like an employee pre-tax contribution of $500. I don’t understand why it’s classified that way.

You'll likely continue to get multiple checks from settlements AFTER you rollover your 401k to another instrument as Stumpf, Sloan, and not Sharf (yeah, how about staying away from the S's) cannot keep themselves or their underlings from creating havoc and operating a dubious company. The lawsuits just keep on coming and Baby Sharf just keeps that $h!t eating grin pasted on his face (I still believe he is the illegitimate son of Klaus Schwab and that white fabric he calls hair is really a rug of biodegradable biopolymers because they are really trying to save the earth.....at the expense of the human race). Du wirst nix besitzen und glücklich sein.
